One “Thanks to You” package for caregivers. Items may include: A waterproof keepsake bag, a caregiver journal, an important phone numbers magnet and caregiver educational material. This VAS is limited to WSCC caregivers who specialize in Long Term Services and Supports (LTSS), Home and Community Based Services (HCBS) and living or moving into a caregiver’s home.
The doula program aims to ease the burden of care access, address cultural barriers, and improve maternal and infant health outcomes. It offers education, focused engagement, and 24/7 on-demand access to virtual doulas.
Treatment for psychiatric symptoms for members who meet ECT medical necessity criteria.
Minor repairs to eyeglasses for members of all ages. Replacement lenses are covered for members of any age if broken or deteriorated, or there is a change in prescription. One (1) pair of eyeglasses (frames and lenses) every 12 months.
Medical and behavioral health benefits, including prescription drug coverage, routine dental and vision services and transportation.
Eligible Members will have access to room and board at the Heading Home’s Respite Care Program Facility with 24- hour care including medication management, individual and group sessions, transportation to appointments, case management focused on discharge planning, financial stability, housing and access to community resources.
Supports whole health that considers the whole person – body, mind, spirit and emotions. The requests can be made by Western Sky Community Care member, staff, providers or caregivers. This VAS is limited to $250, per household.
Effective January 2022 Holistic Care Grant VAS will increase to $250 per household, per calendar year.
Appropriate, non-cosmetic home modifications changes to help members remain independent and safe in their home; Increases length of time spent in the community; improves independent living skills; reduces in-home care; prevents environment related falls.
Expecting Members will receive a car seat or pack and play at no charge. Members are limited to one car seat or pack and play per delivery through the Community Baby Showers.
Reimbursement for ceremonial or spiritual healing that may improve behavioral health and/or physical health. Members are allowed up to $250 per member, per calendar year.
For members who discharge from a PH/BH inpatient setting, Skilled Nursing Facility, Institutional Nursing Facility, Inpatient Rehab and/or LTAC to get to and from the pharmacy to pick up medication. This transportation will be available up to 7 days post-discharge to ensure appropriate discharge medications are filled, and retrieved.
The MyStrength.comExternal Link online program gives Members information to overcome depression and anxiety. This online program includes simple tools, weekly exercises, mood trackers and daily inspirational quotes and videos. The program may be used independently or with other services.
Pyx HealthExternal Link What is the Pyx Mobile app? It is something to support you a little more throughout the day and help you to feel more cared for. It is interactive and will ask you questions like 'how are you feeling?' Based on your answers, it will give you some really great resources and suggestions to help make your day a little better. You can also find resources to support your health, and get help making the most of your health plan benefits.
